Here is a brief checklist to help you prepare for your consultation.

1. Your medical records: These include hospital admission records / discharge summaries, doctors’ notes, blood tests, previous culture reports, and radiologic investigations like x-rays and CT scans.
2. Your current and previous prescriptions: A list of medicines you are currently taking and have taken in the past, especially antibiotics, over-the-counter medicines and oral contraceptive pills.
3. A referral letter from your treating doctor outlining the medical issues / management problems is very helpful.
4. In all cases, I need to examine the patient in person. A medical opinion without examining the patient may be inaccurate. If the consultation is for a hospitalised patient, please request your primary doctor to contact me.
